Completing intake forms does not commit your athlete to receiving care, but intake forms and a chiropractic exam are required before care can be provided.

Schedule your athlete's chiropractic exam.
After the intake forms are complete, Dr. Crowe will contact you to schedule your athlete's exam. Lifeway Chiropractic is a mobile practice. You are responsible for securing a location for your athlete's appointments. Exams may be scheduled individually or with a group of teammates.
